Pennine Lancashire Integrated Care Partnership 2018/2019 Annual report
Date posted: 29th October 2019The Pennine Lancashire Integrated Care Partnership has released their Annual Report for 2018/2019. The report highlights a number of the achievements made over the year as part of the integrated care partnership.
There are a number of challenges faced by health and social care in the Pennine Lancashire. These include economic deprivation, childhood poverty, health inequalities, people living longer with complex care needs, children and young people experiencing poor health outcomes and more people with mental ill-health than anywhere else in the country.
As the Integrated Care Partnership representing East Lancashire Hospitals NHS Trust, Lancashire Care Foundation Trust, East Lancashire CCG, Blackburn with Darwen CCG, Blackburn with Darwen Borough Council, Lancashire County Council, District Councils, and the Voluntary, Community, Faith and Social Care sectors, we are all working together to create a more preventative approach to health and wellbeing for the people of Pennine Lancashire and to tackle the challenges affecting this.
The Annual Report outlines some of the achievements made in our key health and care improvement areas, areas including Prevention and social Prescribing, Neighbourhoods and Community Health Services, Intermediate Care, Regulated Care, Urgent and Emergency Care and Digital Health.
Some of the achievements highlighted for 2018/2019 include the Home First programme supporting over 3000 patients a year, a reduction of the number of care home residents needing to be taken to the hospital, improvements in mental wellbeing for clients attending the Blackburn with Darwen Wellbeing Service and the new GP extended access scheme.
