Home care service gives peace of mind

A service operating across East Lancashire is helping to stop elderly people from being admitted to hospital unnecessarily, or remaining in hospital when they could be cared for at home.

The Intensive Home Support service delivers care for patients in their own home, relative’s home or care home.

The service provides assessment and treatment focussed on a patient’s individual needs in in partnership with existing health, social, voluntary sector and ambulance providers.
